Northamptonshire Healthcare Foundation Trust
Northamptonshire Healthcare Foundation Trust (NHFT) delivers many of the NHS services people in Northamptonshire receive in the community at home, work or in schools. They offer more than 100 services including mental health, community nursing, sexual health, physiotherapy and a range of others including specialty services.
Nene CCG
Nene Clinical Commissioning Group (CCG) is the local commissioning authority for the Northampton locality. GPA is working in partnership with them as a provider, as well as delivering the GP Five Year Forward View through our member practices.
NHS England
Working alongside NHS England to deliver services to the population of Northampton.
NHSE leads the National Health Service (NHS) in England.
Northamptonshire County Council
GPA is working with Northamptonshire County Council (NCC) to deliver patient facing services at Southfields Care Home.
Member practices deliver NHS Health Checks, a national programme offering advice to help prevent the onset of a vascular disease to eligible people, aged between 40 and 74. Public Health England (PHE), the Local Government Association (LGA) and NHS England are working closely together to provide consistent, strong support for the programme.
National Association of Primary Care
NAPC has developed the Primary Care Home
(PCH) model, based on four defining characteristics:
- Provision of care to a defined, registered population of between 30,000 and 50,000.
- An integrated workforce, with a strong focus on partnerships.
- A combined focus on personalisation of care with improvements in population health outcomes.
Lantum
GPA has partnered with Lantum - the healthcare staffing platform that helps primary care providers manage their clinical workforce.
Practices can post available sessions on Lantum and find GPs that are
available to work them. And GPs can browse and book into sessions that suit their
availability.
Northampton Leisure Trust
Northampton Leisure Trust (NLT) operates a number of facilities under the Trilogy brand in partnership with Northampton Borough Council and Duston Parish Council to provide active lifestyles in Northampton.
GPA is working in partnership with NLT to deliver NHS Health Checks and promote wellbeing, as well as actively referring patients to their services through the "Activity on Referral" scheme.
University of Northampton
General Practice Alliance has recently worked with the University of Northampton, facilitating placements within its Collaborative Care Team for final year Occupational Therapy students.
University of Leicester Medical School
GPA runs the Northampton Medical Academy, offering primary care block placements in its teaching practices to third and fifth year students. Students spend structured time within practices, learning consultation skills "on the job", and via group tutorials.
Age UK
Through GPA, Age UK Northamptonshire is working with local GP practices to provide holistic, non-medical support to improve the health and well-being of people living in Northampton.
The Personalised Integrated Care (PIC) programme delivers tailored services and support to older people in Northampton.
Action for Happiness
Action for Happiness is a movement of people committed to building a happier and more caring society. GPA is working with them to promote the GREAT DREAM, via social media and through its practices.
Local Medical Committee - Northampton
Local Medical Committees (LMC) are statutory bodies, recognised by successive NHS Acts as the professional organisation representing individual GPs. The 1999 NHS Act extended the role of LMCs to include representation of all GPs, whatever their contractual status. This includes GPs providing Personal Medical Services (PMS) and non-principal GPs.
