Care Navigation

Phase 1

Care Navigation is a patient-centred approach that uses signposting and information to help Primary Care patients and their carers move through the health and social care system as smoothly as possible.

GPA has up-skilled all receptionists, to Care Navigators in our GP practices in Northampton. This means that when patients call up practices on the day, they will receive a consistent service no matter which surgery they are registered at. 

Care Navigators have been trained to understand the reasons patients are calling and will direct them to the most appropriate source of help. This often means patients will be seen quicker, and doctors will have more appointments for those more complex cases. You may be signposted to;

First for Wellbeing
Community Pharmacy
Practice Nurse / ANP
Physiotherapy
Sexual Health

Phase 2 - Triage Control

Operating directly within SystmOne, Triage Control uses intelligent questioning to enable Care Navigators to gather symptoms and history about a range of clinical conditions. This information allows GPs to make fast and effective decisions about the appropriate course of treatment.
